Optical service means the provision of a sight-correcting appliance upon prescription by a recognised provider, or repair of such repair by a recognised provider.
Recognised provider means a provider who must hold an ABN or a Medicare provider number and be recognised by CBHS Corporate Health in a particular discipline or calling as a provider of services to a member for which CBHS Corporate Health will pay a benefit.
