CBHS Corporate Web Chat is a service established and maintained by CBHS Corporate Health Pty Ltd ACN 609 980 896. You should periodically refer to these Terms of Use as they may be updated from time to time without notice.

This service enables a CBHS Corporate Health Member Care representative to interact and assist you to use the CBHS Corporate Website and Member Service Centre to discuss your health insurance needs and/or respond to any questions you may have regarding your CBHS Corporate Health insurance cover.

Your participation in a CBHS Corporate Web Chat session constitutes acceptance of the CBHS Corporate Website Terms & Conditions, Member Centre Terms & Conditions and CBHS Corporate Privacy Policy which are incorporated by reference in these Terms of Use . 

During the course of your CBHS Corporate Web Chat session with a CBHS Corporate Member Care representative, you may be required to provide us with personal or sensitive information (Personal Information) including:

• (a) Your full name;
• (b) Your residential and/or postal address;
• (c) Your date of birth; and
• (d) Health information relating to you or another person.

By providing us with Personal Information you agree (and have obtained the consent of any other person you provide us with information about) to the transmission of Personal Information at your own risk. 

Any Personal Information you provide during your CBHS Corporate Web Chat session will be securely retained for recording, quality monitoring and training purposes and handled in accordance with the CBHS Corporate Privacy Policy. 

You acknowledge that CBHS Corporate may terminate your use of CBHS Corporate Web Chat if you breach these Terms of Use.
